STUDENT WAIVER, RELEASE, AND CONSENT FORM

Joy Korean Language School

I, the undersigned student (or parent/guardian of a minor student), hereby agree to the following terms regarding my participation in the classes and activities at Joy Korean Language School:

1. General Release of Liability

I understand that while Joy Korean Language School takes reasonable precautions to provide a safe learning environment, accidents or property loss may occur. I hereby release, discharge, and hold harmless Joy Korean Language School, its directors, teachers, staff, and volunteers from any and all claims, liability, or damages for personal injury or property loss arising out of my participation in classes or related activities on the school premises.

2. Medical Emergency Consent

In the event of a medical emergency, I grant permission to Joy Korean Language School and its staff to administer basic first aid and/or seek emergency medical treatment for me (or my child). I understand that the school will make every effort to contact the designated emergency contact person immediately. I assume full responsibility for any medical expenses incurred.

3. Photo and Media Release

I grant Joy Korean Language School the right to take photographs, video, or audio recordings of me (or my child) during classes or school events. I authorize the school to use these materials for educational, promotional, and marketing purposes, including on the school's website and social media, without compensation.

I HAVE READ THIS WAIVER, RELEASE, AND CONSENT FORM, FULLY UNDERSTAND ITS TERMS, AND SIGN IT FREELY AND VOLUNTARILY.
